Treatment for ovarian cancer is frequently a combination of cytoreductive surgery and chemotherapy. Surgical treatment is intended to remove the tumor as well as any nearby tissues where the cancer has spread. In addition to a localized tumor, ovarian cancer frequently includes micrometastases that have spread from the ovaries to other parts of the body. Because surgery is a local treatment, it cannot treat micrometastatic cancer cells. As a result, either before or after cytoreductive surgery, also known as debulking surgery, a systemic treatment, such as chemotherapy, may be used. Chemotherapy involves the use of powerful drugs that can enter the bloodstream to reach and destroy widespread cancer cells. The most common type of cytoreductive surgery used to treat ovarian cancer is an exploratory laparotomy, also known as upfront cytoreductive surgery. During a laparotomy, a gynecologic oncologist can accurately diagnose and stage a tumor (the extent of tumor spread), remove as much cancer as possible from the abdominal and pelvic regions, and determine whether additional therapy is necessary.
